How to Purchase Organisms and Cell lines from ATCC
All ATCC products must be purchased from Cedar Lane.
Place order via UW Procurement and Contract Services to Cedar Lane.
You will receive an e-mail with the following forms attached
- CDR 111 PHAC/CFIA End-User Notification and Confirmation Letter
- Waiver CDR 160, Authorization a Cedar Lane access to PHAC/CFIA Check lists
- MSDS sheets(For your reference)
- PHAC Check List (required for human pathogens) Not required if a current PHAC compliance letter is on file with the institutional Bio-Safety Officer
- CFIA Check List(required for animal pathogens) Not required if a current CFIA compliance letter is on file with the institutional Bio-Safety Officer
- ATCC ‘MTA’ (material Transfer agreement
- Acknowledgement of ATCC MTA to to Cedar Lane
Forward forms to the Institutional Bio-Safety Officer Ian Fraser
The institutional Bio-Safety Officer will complete the forms, forward them to appropriate agencies.
How to Import Animal or Human Pathogens
- researcher completes either or both
- Application to Import Animal Pathogens
- Application to Import Human Pathogens
- UW Bio-Safety Officer completes one or both
- All documents and fees are sent into various agencies by the institutional Bio-Safety Officer
